Avita Health System is offering reduced-cost blood screenings at several of its locations across the region. These screenings are available through a voucher system and will remain open through October 31.
How to Participate
To take part, participants must first purchase a voucher in person. Vouchers are available at:
- Galion Hospital cashier’s office
- Registration desks at Bucyrus and Ontario Hospitals
- Bellville and Shelby Walk-In Clinics
Note: Vouchers are not available at the Ontario South Campus or the Ontario Walk-In Clinic.
Available Screenings and Prices
- HealthLink Lab Profile (Lipids, Chems, CBC) – $30
- PSA (Prostate-Specific Antigen) – $25
- HgbA1c (Glycosylated Hemoglobin) – $30
- TSH (Thyroid Stimulating Hormone) – $15
- Vitamin D, 25 – $30
You can redeem vouchers during October at any Avita Hospital lab, the Ontario South Campus lab, or the Bellville or Shelby Walk-In Clinics. If you meet the testing prep requirements, you may even purchase and redeem your voucher on the same day.
Important Details
- Vouchers are only valid through October 31, 2025
- Results will be mailed directly to participants
- Your provider will not automatically receive a copy, so it’s your responsibility to share the results
